Elder Connections is a unique home care company that provides integrated home care to help seniors remain at home. Utilizing a multidisciplinary team of professionals, a care plan is designed specifically for the client and their family. Geriatric care managers and nurses are involved with tailoring care to the individual’s needs, supervising that care, while interfacing with the family throughout the process. Care managers can also help families make decisions about the course of care. We counsel and coach families about alternative living arrangements, professional resources such as physicians, elder law attorneys, etc. and provide insight into dealing with the process of aging.
